Sample Therapist's Contract

This is a Word file that contains a sample, basic contract that you might use as a starting point for developing a formal written agreement between you and your child's ABA Therapists. We have left the file in Word format so that you can change it easily to suit your needs.

Sample letter of medical necessity for ABA services

This is a sample letter in Word format that you can bring to your child's pediatrician and ask them to sign. The letter is basically a prescription for ABA services that you can keep in your files for tax or insurance purposes. In addition to the a prescription, the letter also includes references to research articles and independent reviews of research all showing that early and intensive ABA for young children with autism is warranted and effective. We have left the letter in Word format so that you or your child's pediatrician can easily modify it.
